About Your Visit
We accept most major insurance plans. Please remember to bring your insurance card with you to your appointment. Co-payments are due at time of service.
Workers Compensation: In order to bill your workers compensation claim, we need the full name and address of the workers compensation carrier, date of injury, “Employer’s report of occupational injury” and address and phone number of your employer and contact person.
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Emergency: If you have a true medical emergency after office hours, please call (724) 775-4774.
Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, and Discover for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
You may need a referral from your Primary Care Physician (PCP) to see one of our physicians. It is the patient's responsibility to get this referral. Please refer to your insurance booklet you received from your carrier or employer. Also, there may be a phone number listed on your insurance card for contact information about your insurance plan. If you do not have a required referral, we will need to reschedule your appointment.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals between 8:00am — 3:00pm, Monday to Thursday and 8:00am — 12:00pm Friday, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
Should you have questions concerning your bill from the practice, please call (724) 774-6445.
Please remember to bring any previously taken x-rays films, MRIs, CT scans, and myelograms done by a facility other than The Medical Center, Beaver to your appointment. All bone scans films regardless of where they were performed must be brought with you.
Disability and Insurance Forms
There is a $20.00 charge for most disability and insurance forms to be completed.

For Your Appointment
If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:
- Driver's License or State ID
- Referral (If Required by Insurer)
- Insurance Information
- Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits
- A List of Current Medications and Any Known Allergies
